1st visit $100 cash/credit/insurance (if accepted) Follow up appointment $75 (High deductible insurance is charged $40 at time of visit)

Cancellation Policy

Patients have 24hrs before appointment to cancel. If canceled after 24 hrs and not approved with staff patient will be charged $30.00.
