WHAT TO EXPECT
For the protection of all our patients, when you visit Aspen Family Medicine, you will be pre-screened for entry. Patients (and their escort) will have their temperature taken when they arrive. If their temperature is 100 degrees or higher, or they answer yes to any of the following questions, they will not be allowed to enter the clinic, but will receive further instructions on how to proceed.
Pre-Screening Questions
Have you or anyone living in your home had contact with anyone confirmed or under testing for COVID 19 in the past 14 days?
Do you or anyone living in your home currently have or have had a fever over 100 F in the past 14 days?
Do you or anyone living in your home currently have or have had any fever symptoms such as chills or excessive sweating in the past 14 days?
Do you or anyone living in your home have a cough?
Do you or anyone living in your home have any shortness of breath not related to a chronic medical condition?
Have you or anyone living in your home have any new loss of smell or taste?
Have you or anyone living in your home had any muscle pain, headache or sore throat?
Have you or anyone living in your home been notified by the Health Department, your employer or school to self-quarantine?
All patients are required to wear a mask. To preserve our supply of masks, we ask you to bring your own mask to your appointment (cloth DIY masks are acceptable). Otherwise, one will be provided for you.
